This discussion will be lead by Carolyn Stramecki BS, MHSA, CPHQ, who is the Quality Improvement Coordinator for the Care Transitions team and an Improvement Advisor for the Michigan State Action on Avoidable Rehospitalizations (MI STA*AR) initiative. For the past 3 years Ms. Stramecki coordinated efforts with the Institute for Healthcare Improvement (IHI) and hospitals participating in the MI STAAR initiative. More recently she has joined MPRO's care transition team in which she assists communities in building coalitions and improving and coordinating transitions in care.

DescriptionBasic directions: Exit US 127 at the Jolly Road exit (1st exit north of I-96 intersection). At the end of the exit ramp turn left (east) on Dunkel. Turn left (north) on Collins Road. In approximately one mile, turn left (west) on Forest Road. The Henry Center for Executive Development and Candlewood Suites is located on the immediate right. For more detailed directions, please see the drop down menu below. From Downtown Lansing: Take I-496 East, which will merge with US-127 South. Stay on US-127 South and take the Jolly Road exit. At the end of the exit ramp turn left (east) onto Dunkel. Go approximately ½ mile and turn left (north) at the 'T' intersection onto Collins Road. As Collins Road begins to curve to the right, turn left (west) on Forest Road. The Henry Center, Candlewood Suites, Forest Akers Golf Course and the University Club will be on the immediate right.
